according to my experience the following causes of loss for usually traders.

1. start real trading without any training, learning and practice on demo trading account

2. uncontrolled emotions and greed

3. lake of knowledge about forex rules and regulation

4. lake of risk and money management abilities

5. lake of patience

I think the main reason behind loss is lack of knowledge most of the traders don't try to get information about Forex market and they don't practice Forex trading on demo account and they go for real trading in a hurry that is the reason they get loss in Forex trading.
